First Communion Registration fees:

Payment may be made by cash, check, or credit card in one or multiple payments. You will have the opportunity to select your preference at the end of the registration form.

Please print and sign the Minor Consent/Liability Release Form, and return it to the Family Life & Formation office.

El pago puede realizarse en efectivo, cheque o tarjeta de crédito en uno o varios pagos. Tendrá la oportunidad de seleccionar su preferencia al final del formulario de inscripción.

Imprima y firme el formulario de consentimiento/exención de responsabilidad para menores y devuélvalo a la oficina de Vida Familiar y Formación.
